Ontario Health atHome is committed to protecting your privacy and implementing technology
that gives you an effective and safe online experience. This Privacy Policy
applies to this Quadrant Self Service (QSS) Web application and governs the
collection, use and disclosure of personal information through this site.
This Privacy Policy explains how personal information about you may be
collected, used or disclosed as a consequence of your activities on the Ontario

By using Quadrant Self Service (QSS), providing your personal information

Ontario Health atHome is not a commercial or private organization our privacy

Collection of your Personal Information
Ontario Health atHome collects personally identifiable information through
Quadrant Workforce, such as your e-mail address, name, home or work address or
telephone number, benefits and leave information for the purposes of operating
the web application and delivering the required services.
Personal information that we collect may include, but is not limited to:
Pay statements; staff can view and print their own pay statements
Personal information contained in material that you elect to post
to this Site
Responses to leave management requests
Information collected to register as a user of this Site (Registration
Information). Registration Information is collected for the purpose of identifying
you as a user of this Site
Benefit Banks associated with your own banked hours and other
banked items
Competencies staff current skill sets or competencies
There is also information about your computer hardware and software that is
automatically collected by QSS. This information can include: your IP address,
browser type, domain names, access times and referring Web Site addresses. This
information is used by QSS for the operation of the service, to maintain
quality of the service, and to provide general statistics regarding use of QSS.
Ontario Health atHome is not responsible for the privacy statements or other
content on Web Sites linked or outside of Ontario Health atHome QSS web
application and Ontario Health atHome family of Web Sites.
Use of your Personal Information
Ontario Health atHome uses your personal information to operate the web
application and deliver staff services. We use your personal information in
order to provide your pay statement, benefit banks and competencies, administer
and process scheduling, and leave management.
Ontario Health atHome may contact you via surveys to conduct research about
your opinion of current services or of potential new services that may be
offered.
Health Shared Services Ontario will have access to QSS for purposes of
administering and supporting the system on behalf of the Ontario Health atHome.
Staff using the Site may be known to both the Ontario Health atHome and Health
Shared Services Ontario who administers the Site.
Disclosure of your Personal Information
Ontario Health atHome will disclose your personal information, without notice,
only if required to do so by law or in the good faith belief that such action
is necessary to: (a) conform to the edicts of the law or comply with legal
process served on Ontario Health atHome or the site; (b) protect and defend the
rights or property of Ontario Health atHome; and, (c) act under exigent
circumstances to protect the personal safety of users of QSS, or the public.
Use of Cookies
QSS uses "cookies" to help you personalize your online experience. A

that you not share it with anyone, in order to safeguard your personal
information on our Site.

Although we use all reasonable means to ensure that the information you provide

Policy, the Ontario Health atHome is not responsible for any improper use of
your personal information that is beyond our reasonable control.
