Protection of Personal Information

While the collection, use or disclosure of personal information through this site is not subject to Canadian or provincial privacy legislation, Champlain LHIN follows the ten privacy principles set out in the National Standard of Canada Model Code for the Protection of Personal Information (which forms Schedule 1 to the Personal Information Protection and Electronic Documents which forms the basis for the protection of personal information in Canada. The Champlain LHIN is based in Ontario, Canada.
The types of personal information that is protected includes almost any information about an identifiable individual, such as your name, address, telephone number, age, sex, marital status, and nationality, if provided by you. Personal information may not, however, include certain types of business or employment related information.
Champlain LHIN does not collect, use or disclose your personal information without your consent, except where required or permitted by law.
Champlain LHIN does not sell, rent or lease its staff lists to third parties.
In addition to the provisions of this Privacy Statement, your use of this Web Site is governed by the QSS Terms and Conditions on this site.

Collection of your Personal Information

Champlain LHIN collects personally identifiable information through Quadrant Workforce, such as your e mail address, name, home or work address or telephone number, benefits and leave information for the purposes of operating the web application and delivering the required services.
Personal information that we collect may include, but is not limited to:
     Pay statements staff can view and print their own pay statements
     Personal information contained in material that you elect to post to this Site
     Responses to leave management requests
     Information collected to register as a user of this Site (Registration Information). Registration Information is collected for the purpose of identifying you as a user of this Site.
     Benefit Banks associated with your own banked hours and other banked items
     Competencies staffs current skill sets or competencies
Champlain LHIN may also collect demographic information, which is not unique to you, such as your postal code, age, and gender.
There is also information about your computer hardware and software that is automatically collected by QSS. This information can include: your IP address, browser type, domain names, access times and referring Web Site addresses. This information is used by QSS for the operation of the service, to maintain quality of the service, and to provide general statistics regarding use of QSS.
Champlain LHIN does not use web beacons to track users of this Site.
Champlain LHIN is not responsible for the privacy statements or other content on Web Sites linked or outside of Champlain LHINs QSS web application and Champlain LHINs family of Web Sites.

Use of your Personal Information

Champlain LHIN uses your personal information to operate the web application and deliver staff services. We use your personal information in order to provide your pay statement, benefit banks and competencies, administer and process scheduling, and leave management.
Champlain LHIN may contact you via surveys to conduct research about your opinion of current services or of potential new services that may be offered.
Health Shared Services Ontario (HSSO) will have access to QSS for purposes of administering and supporting the system on behalf of the Champlain LHIN. Staff using the Site may be known to both the Champlain LHIN and HSSO who administers the Site.
Champlain LHIN keeps track of the Web pages its users visit within the QSS web application in order to determine what QSS services are the most popular. This data is used to deliver customized content to users whose behavior indicates that they are interested in a particular subject area.

Disclosure of your Personal Information

Except as required by law or with your consent, Champlain LHIN does not disclose your personal information.

Access and Accuracy of your Personal Information

Upon a request made in writing Champlain LHIN will inform you of the existence, use and disclosure of your personal information, at no cost to you in accordance with Champlain LHIN Release of Information Policy. Champlain LHIN will respond within 30 days of the request or as otherwise required or permitted by law.
An individual shall be able to challenge the accuracy and completeness of the information and have it amended as appropriate.

Use of Cookies

QSS uses "cookies" to help you personalize your online experience. A cookie is a text file that is placed on your hard disk by a Web page server. Cookies cannot be used to run programs or deliver viruses to your computer. Cookies are uniquely assigned to you, and can only be read by a Web server in the domain that issued the cookie to you.

For example, if you personalize QSS pages, or register with the QSS Site or services, a cookie helps QSS to recall your specific information on subsequent visits. Security of your Personal Information

Champlain LHIN has adopted physical, organizational and technological measures to safeguard your personal information. The Champlain LHIN through QSS secures your personal information from unauthorized access, use or disclosure on computer servers in a controlled, secure environment.
Champlain LHIN employees and agents are subject to contractual obligations regarding the protection of personal information. Champlain LHIN makes every effort to ensure that access to personal information is granted only to those individuals who need the information for the identified purposes.
It is important that you keep your User ID and password information secure and that you not share it with anyone, in order to safeguard your personal information on our Site.
There is occasionally fraudulent phishing emails targeting individuals who they believe might be registered users of this Site. Champlain LHIN never sends emails which requests users to visit a web page or place a call and provide their user ID, password or any personal information. We will never ask you to confirm your user ID or password via email. If you receive such an email do not log in from any referred link and do not provide any personal information. You should contact our Privacy Officer at 613.745.8124. Our employees receive regular privacy training. Employees are instructed to only use personal information in adherence with the principles set forth in this Privacy Policy and applicable laws. Employees who misuse personal information may be subject to disciplinary action.

Improper Use of Personal Information

Although we use all reasonable means to ensure that the information you provide to us is not used for purposes other than those described in this Privacy Policy, Champlain LHIN is not responsible for any improper use of your personal information that is beyond our reasonable control.

Retention of Information

In accordance with the record retention practices of the Champlain LHIN, personal information is only kept until it is no longer necessary or relevant for the identified purposes, subject to retention periods or legal requirements.
