North East LHIN is committed to protecting your privacy and implementing technology that gives you an effective and safe online experience.

This Privacy Statement applies to the Quadrant Self-Service (QSS) web application and governs the collection, use and disclosure of personal information through QSS. It also explains how your personal information may be collected, used or disclosed as a consequence of your activities within the QSS application. We urge you to read it carefully.

Providing your personal information through the QSS application you are deemed to consent to the collection, use or disclosure of your personal information for the purposes described below in accordance terms of this Privacy Statement.

North East LHIN protects the personal information it collects as required by applicable Canadian or provincial privacy legislation. It also follows the ten privacy principles set out in the Canadian Standards Association's Model Code for the Protection of Personal Information.

North East LHIN does not sell, rent or lease its personal information to third parties.

In addition to the provisions of this Privacy Statement, your use of this Web Site is governed by the QSS Terms and Conditions on this site.

Collection of your Personal Information

North East LHIN collects your personal information directly from you and stores it in the QSS Application. This information may include but is not limited to:

• Age
• Sex
• Marital status
• Nationality
• Address (home and work)
• Telephone number(s)
• Email address(es)
• Employment benefits
• Leave requests and responses
• Information about your computer hardware and software such as your IP address, browser type, domain names, access times, and referring Web Site addresses.

North East LHIN does not use web beacons to track users of this Site.

North East LHIN is not responsible for the privacy statements or other content on Web Sites linked to or outside of North East LHINs QSS web application and North East LHINs family of Web Sites.


Use of your Personal Information

North East LHIN uses personal information in the QSS Application for the purposes of operating the web application, delivering services, improving the quality of services and monitoring statistics related to the use of the services.

 

For example, we use your personal information in order to:

• Provide you access to your pay statement and benefit banks
• Accept and process leave and scheduling requests
• Monitor what web pages you visit within the web application (so that we can deliver customized content to you in the future).

North East LHIN may contact you via surveys to conduct research about your opinion of current services or of potential new services that may be offered.

North East LHIN and Health Shares Services Ontario (HSSOntario) are jointly responsible for administering the QSS application and its related services. As a result, your information may be viewed by staff from North East LHIN or HSSOntario for the purposes specified above.


Disclosure of your Personal Information

North East LHIN will not collect, use or disclose your personal information without your express consent except where required or permitted by law.


Access and Accuracy of your Personal Information

North East LHIN will provide you with access to your personal information upon receipt of a written request.

You have the right to challenge the accuracy and completeness of your personal information and to request (in writing) that it be amended.

Use of Cookies

QSS uses cookies to help you personalize your online experience. A cookie is a text file that is placed on your hard disk by a Web page server. Cookies cannot be used to run programs or deliver viruses to your computer. Cookies are uniquely assigned to you and can only be read by a Web server in the domain that issued the cookie to you.

One of the primary purposes of cookies is to provide a convenience feature to save you time. The purpose of a cookie is to tell the Web server that you have returned to a specific page. For example, if you personalize QSS pages or register with the QSS Site or services, a cookie helps QSS to recall your specific information on subsequent visits. This simplifies the process of recording your personal information. North East LHIN makes every effort to ensure that access to personal information is granted only to those individuals who need the information for the identified purposes.

North East LHIN employees and agents have legal obligations regarding the protection of your personal information. The employees receive regular privacy training and are aware that unauthorized access or inappropriate use of personal information may result in disciplinary action.

In order to safeguard your personal information on our Site, it is important that you keep your User ID and password information secure and that you not share it with anyone.

There are occasionally fraudulent phishing emails targeting individuals who are believed to be registered users of this Site. North East LHIN never sends emails which requests users to visit a web page or place a call and provide their user ID password or any personal information. We will never ask you to confirm your user ID or password via email. If you receive such an email:

• Do not log in from any referred link;
• Do not provide any personal information; and,
• Contact the individual identified under the Contact Information section below.
