There is also information about your computer hardware and software that is automatically collected by Quadrant Self Service. This information is used by Quadrant Self Service for the operation of the service, to maintain quality of the service, and to provide general statistics regarding use of Quadrant Self Service.

HNHB LHIN is not responsible for the privacy statements or other content on Web sites outside of HNHB LHINs Quadrant Self Service web application and HNHB LHINs family of Web sites.

Quadrant Self Service collects and uses your personal information to operate the web application and deliver the services you have requested. HNHB LHIN may contact you via surveys to conduct research about your opinion of current services or of potential new services that may be offered.

HSSOntario will have access to Quadrant Self Service for purposes of administering and supporting the system on behalf of the HNHB LHIN.

HNHB LHIN does not sell, rent or lease its contact lists to third parties.

HNHB LHIN does not use or disclose sensitive personal information, such as race, religion, or political affiliations, without your explicit consent.

HNHB LHIN controls access to the Quadrant Self Service system themselves and each user will only have access to their own information. Terminated employee access will be removed immediately from Quadrant Self Service and would be required to contact Human Resources should they need access to their payroll information.

Quadrant Self Service keeps track of the Web pages its users visit within the Quadrant Self Service web application, in order to determine what Quadrant Self Service services are the most popular. This data is used to deliver customized content to users whose behavior indicates that they are interested in a particular subject area.
